Building Bridges Between Healthcare and Social Care to Transform Lives
New industry white paper from GroundGame.Health and Sage Growth Partners explores solutions to transform the American healthcare ecosystem

Tampa, FL, February 13, 2025 – GroundGame.Health has partnered with Sage Growth Partners (Sage), a healthcare growth strategy and marketing firm with deep expertise in market research, go-to market strategy, and marketing communications, to release new white paper insights on transforming the healthcare ecosystem.
Building Bridges Between Healthcare and Social Care to Transform Lives, offers a deep dive into the importance of meeting people where they are. In 2024, Sage conducted a survey of consumers who indicated they had experienced a gap in social needs during the previous 5 years. This research underscores the urgency and opportunity ahead for solving social care needs such as food, transportation, housing and caregiving.
Of those surveyed, nearly half indicated they have had trouble obtaining food or clothing when needed and one-third stated that have had difficulty finding a doctor, scheduling a medical appointment, or obtaining necessary medications. These findings reiterate that even with proper support networks in place, people still frequently struggle to access basic necessities across both the “health” and “social” spectrum.
“The benefits of solving both social and healthcare needs are an important signal to the market that we shouldn’t be solving for high costs and poor outcomes in isolation of one another. The challenge is finding a way to collaborate among and between all stakeholders that is efficient, effective, and sustainable,” said Susan Rawlings Molina, co-founder and CEO, GroundGame.Health.
The white paper offers frontline perspectives on the impact that proper funding, technology, regulatory frameworks and public partnerships can have on transforming lives.
“People need more than just programs, they need advocates who can help them navigate the care system and available offerings,” said Dan D’Orazio, CEO, Sage Growth Partners. “Even if we find a way to solve all of the challenges, the most well-funded and innovative initiatives can not transform people’s lives if there is no trust between patients, caregivers and health plans.”
