About our solutions

At GroundGame.Health, we manage the complex connections between health plans, providers, employers, community-based organizations (CBOs), and other stakeholders to fulfill unmet health-related social and care needs for people across the country. By facilitating personalized, culturally tailored, human-to-human interactions, we make the biggest difference in people's lives.

Our scalable, hyperlocal approach is driven by our proven technology platform and national network of CBOs. Through our unique model, we increase financial capacity within communities to sustain social care delivery, making it easier for health plans and CBOs to fulfill their shared goal of closing the loop on social needs and gaps in care.

How we’ve worked with our partners to make a difference

Real people, real stories.

An older man with gray hair, standing, while holding onto a rollator.

Randy*

Randy, 75, had mobility issues, as his rollator was both too small, and in disrepair. Also, his caregiver was unreliable, and his cell phone was not working — making it impossible to seek help. Our care coordinator intervened, arranging a new rollator prescription, and helping source a new one. She intervened with the care agency on his behalf, and also arranged for a new phone. The support has led to a much better quality of life for Randy.

Jenny*

Jenny is a 58-year-old woman who despite her employment status, struggles from time-to-time keeping food on the table, and paying her utilities during the cold weather months. We helped Jenny sign-up for food support that she qualified for, and connected her with a local resource that provides financial assistance with her utility bills.

An older Hispanic man who appears to be concerned or worried.

José*

José, 65, is diabetic, but was having trouble affording his prescribed medication and testing equipment. As a result, he was not feeling well, with elevated A1c values. With our care coordinator’s help, José was connected to a program to help pay for his medication and testing gear. The coordinator even followed up with José's PCP to see if more help was needed. Today, José’s diabetes is well-controlled.
