WHO IS GGH?

GroundGame.Health helps people like you to get the support they need to live their healthiest lives. We work with health plans, physicians, and employers to identify those who may need support. Then, in partnership with community-based organizations across the country, we help them find and use available resources, sign-up for programs that can provide needed assistance and ensure that both their short-term and long-term needs are met.

Why did I get a call?

Did you get a call from GroundGame.Health, but aren't sure why?

Your health plan, your employer's human resources department, or your physician may have asked us to check with you and see if you need help with a health-related issue, or life challenge such as finding a place to live, paying for medicine, getting rides to the doctor, or fixing a problem at home.

When we call you, please take a moment to talk to us. We can explain why we were asked to contact you, and see if there are any ways for us to help you. And, if now is not a good time, we can schedule another time that works better for you. You can also complete the form below to get in-touch.

Our services are provided at no cost to you, and our conversations are completely confidential.*

One size does not fit all

Different people have different needs, that impact them in different ways. Whether you're experiencing health concerns such as paying for your medications, or life challenges such as unsafe living conditions, GroundGame.Health may be able to help you.* Here are just some of the ways:

  • Behavioral health needs
    Locating mental health support services.
  • Caregiving
    Help caring for family members.
  • Childcare
    Finding daycare or after-school programs.
  • Education support
    Connections to programs for learning English, getting a high school equivalency certificate (GED), etc.
  • Food
    Getting help with food, and signing-up for programs like SNAP and WIC.
  • Home environment
    Fixing problems at home, like mold, pests, or heating.
  • Housing and unsafe living conditions
    Finding a place to live — or a safer living environment.
  • Legal support
    Locating legal help to address advance directives, powers of attorney, bankruptcy, etc.

    Obtaining discounts on medications, or assistance with copays.

    Support with bills such as utilities, rent, or credit cards; finding help with debt.

    Finding someone to spend time with, or joining support groups.

    Quitting smoking, drinking, or other unhealthy habits.

    Arranging rides to medical appointments, or errands like grocery shopping.

José**

José, 65, is diabetic, but was having trouble affording his prescribed medication and testing equipment. As a result, he was not feeling well, with elevated A1c values. With our care coordinator’s help, José was connected to a program to help pay for his medication and testing gear. The coordinator even followed up with José's PCP to see if more help was needed. Today, José’s diabetes is well-controlled.

Maria**

Maria is a 45-year-old woman who speaks Spanish, and needed someone to talk to about sometimes not having enough food for herself and her family. She also had trouble paying her rent and bills. GroundGame.Health helped Maria enroll in a local program to help pay her rent and bills, and found a food pantry that Maria could access when needed.

Kevin**

Kevin, 49, suffers from PTSD, and was homeless when our care coordinator reached out. Kevin needed housing, food, and medication support. Our coordinator helped first by ensuring that he had help with food and fuel to stay warm and fed, and arranged support with medications. They then worked to get Kevin on a waiting list for housing, which he eventually secured, ensuring his longer-term well-being.
