Solving people’s social needs

Our roots are in community-based work, and our leadership has deep expertise in the healthcare sector. We were founded as part of a Community-Based Organization (CBO) in Indiana — so we understand the challenges that CBOs face from firsthand experience.

In the years since, as we’ve expanded our model nationwide and launched our Implify™ platform, we’ve prioritized the needs of CBOs every step of the way.

We work to expand the reach of CBOs with reliable expertise, and configurable, easy-to-use technology. Ultimately, our innovative model and interoperable platform gives you more time and sustainable funding to do what you do best: help more people, on the ground.

Sarah*

Sarah is a 50-year-old woman who had difficulty paying for medications, and for food. After we helped Sarah, she said, "GroundGame.Health connected me with a discount program that made my prescriptions more affordable. And they arranged help with my groceries when I needed it."

Todd*

Todd, 62, is disabled, and had multiple resource needs — most importantly with food, since he often went hungry when SNAP alone didn't fully meet his needs. Our care coordinator set to work, arranging with a local organization to regularly deliver meals to Todd at home. He’s grateful that he’s received the food support he needs to ensure his well-being.

David*

David is a 53-year-old man whose mother, who lives alone, was recently diagnosed with Stage 4 cancer. As her only source of support, David was overwhelmed by his mother's needs, and the emotional impact they had on him. GroundGame.Health connected David with local resources to support him in coordinating his mother's end-of-life needs, and found grief support resources he could he could rely on.

Our partnership with USAging

GroundGame.Health is proud to have a partnership with USAging, the national association representing Area Agencies on Aging (AAA) across the country. AAAs are at the forefront of the effort to help older adults and people with disabilities improve their health, maintain independence, and live with dignity in their homes and communities.

GroundGame.Health is honored to work closely with USAging as a strategic partner, as we build more resilient communities and close more gaps in care fort his population. Importantly, we also connect AAAs to sustainable sources of revenue to fund their work.
