How we work
GroundGame.Health has developed a business model that brings diverse stakeholders, processes, terminologies, technologies, and payment structures together, to connect people to the community-based organizations and resources they need on the ground, to overcome the barriers that are preventing them leading healthier lives.
Whether a person needs transportation, food, housing, help applying for benefits, or another needs, we have the boots on the ground to support them.
Through human-to-human connections, we build trust through compassion and empathy with each person we engage. That trust enables us to uncover and solve the most important obstacle each person is facing.
1: Population
We begin by identifying the population of individuals who currently have unmet social needs.
2: Integrations
Our bidirectional integration capabilities with customers allows us to exchange information such as referrals, care plans, claims, etc.
3: Implify™
The Implify™ platform intelligently routes information to relevant resources, and documents both the activities performed, and the results achieved.

4: Help
GroundGame.Health staff, along with community-based organizations and/or resource providers, work directly with the people who need help — on the ground.
5: Document
The Implify platform creates a secure, cohort-based structure to assess and capture health-related social needs, documents gaps that are closed, as well as the resources that were ultimately provided.
6: Financial
The Implify platform enables both upstream and downstream billing and payments, through invoices and/or claims.

Sarah*
Sarah is a 50-year-old woman who had difficulty paying for medications, and for food. After we helped Sarah, she said, "GroundGame.Health connected me with a discount program that made my prescriptions more affordable. And they arranged help with my groceries when I needed it."
Maria*
Maria is a 45-year-old woman who speaks Spanish, and needed someone to talk to about sometimes not having enough food for herself and her family. She also had trouble paying her rent and bills. GroundGame.Health help Maria find a place nearby where she could get help paying her rent and bills, and found a food pantry and Maria could access when she needed.
David*
David is a 53-year-old man whose mother, who lives alone, was recently diagnosed with Stage 4 cancer. As her only source of support, David was overwhelmed by his mother's needs, and the emotional impact they had on him. GroundGame.Health connected David with local resources to support him in coordinating his mother's end-of-life needs, and found grief support resources he could he could rely on.
